Decide what service(s) you are interested in (individual counseling, couples counseling or group counseling). Counseling sessions are 50 minutes and group classes vary in length. We have day, evening and Saturday appointments available (Mon-Fri 9:10am to 9:10pm, Sat 9:10am -6:10pm).
Step 2-Fees
We take insurance. Call us to see if your insurance company plan is in our network. If you do NOT have insurance, we offer a sliding scale fee based on yearly family income. Review the sliding scale fee to determine cost. Forms of payment accepted – cash and credit cards (NO Checks).

Arrive 20 minutes early for first appointment to fill-out paper work and bring proof of income and payment. OPTIONAL-- You can download initial paperwork, print and fill-out at home and bring to first appointment

1-Professional Counseling Agreement– this form is our standard counseling agreement between New Day Counseling and the person receiving therapy. Please read carefully and sign where indicated (for couples counseling each adult needs to fill out this form)
5-Life History - Adult – this form gives your therapist an overview of your life history that will help establish goals and develop a treatment plan that is best suited to your needs. This form is for adults (for couples counseling each adult needs to fill out this form)

Life History - Child/Adolescent – this form is for an overview of a minor’s history to be filled out by a parent of a child/adolescent coming in for counseling
Minor Release Form - each parent needs to print and sign this form (one form per parent)
